Items of note:
1.  For those new to the system, 'Photomates' are individual and team pictures, buttons and magnets of your student athlete. The 8x10" composite includes a team photo and your individual student athlete photo. At the high school level, 'Specialty Composites' are available that feature multiple shots of your student athlete in competition.

2.  Please complete a separate form for each athlete.

3.  ATHLETES PLAYING MORE THAN ONE LEVEL: If you would like to purchase photos for more than one level, please complete a separate form for each level.

4.  PAYMENT INSTRUCTIONS: By submitting this form, you agree to pay for the order placed. Please make checks payable to Aquin Catholic Schools and return in an envelope clearly marked "Photomate Payment" to the Collections Box in the Student Office.  Please return your payment promptly once the form has been submitted.
