1. Please complete a separate form for each athlete.
2. ATHLETES PLAYING MORE THAN ONE LEVEL: If you would like to purchase photos for more than one level, please complete a separate form for each level.
3. PAYMENT INSTRUCTIONS: By submitting this form, you agree to pay for the order placed. Please make checks payable to Aquin Catholic Schools and return in an envelope clearly marked "Photomate Payment" to the Collections Box in the Student Office (Elementary Parents may submit payment to Mrs. Grenoble or Mrs. Diemer). Please return your payment promptly once the form has been submitted.
Junior High Boys, High School Boys and High School Girls Orders AND money must be submitted and turned in by Monday, December 1, 2008 to avoid a late fee ($5 per order). Junior High Girls, 5th-6th Boys and 5th-6th Girls will be due at a date yet to be determined.
4. NEW THIS YEAR: SPECIALTY COMPOSITES are now available for high school athletes only. We will create a personalized montage of the athlete using pictures from the season, photoshopping techniques and other tech goodies. The final product will be an 8x10" print and will vary by sport and individual.
