School Insurance Information
Below you will find a link to information regarding the supplemental insurance that is being offered through the school.This insurance is available to every student in the system. Please take a look at the information, complete this form and return it to the Business Office at your earliest convenience whether you will be enrolling in a plan or not. It is important that we receive a form back from every family by August 20th.
Please be aware that if your child will be participating in one of the Aquin Catholic Schools' sponsored extra-curricular activities, the student must be covered by some type of medical insurance plan.
Also, all athletes must show proof of insurance before the first day of practice for that sport. No one will be allowed to practice unless we have that information on file. Please remember too, that all athletes must have a current physical on file with the school before they can begin practice.

Cub Foods Update
As many of you may have read in the paper this week (or heard through the grapevine), Cub Foods is discontinuing their 'Community Dollars' program. We received a letter from Store Director Kurt Steffen addressing this issue. Per this letter, we were informed that the program will be discontinued September 5, 2008 for most organizations.
But, they have extended the deadline until November 28th for their top 10 groups. We are one of their top 10 groups! So, keep saving those receipts. Second quarter receipts need to be turned in by September 5th and third quarter receipts need to be turned in by November 28th.
Cub Foods will continue their other community programs, such as 'Cub Cares Community Brat Stand', gift card donations and gift card cost reductions (Scrip).

STATE OF ILLINOIS MANDATES FOR THE FALL OF 2008
The State of Illinois mandates that all children entering for the first time preschool, kindergarten, SIXTH, and ninth grades have a school physical and update age-appropriate immunizations. ENTERING FIFTH GRADERS ARE NO LONGER ARE REQUIRED TO HAVE PHYSICALS. FOR THIS YEAR ONLY, CURRENT FIFTH GRADERS WILL NOT BE REQUIRED TO HAVE ANOTHER PHYSICAL BEFORE ENTERING SIXTH GRADE.
Moreover, state law now requires a screening for diabetes be completed on all school physicals and a lead risk assessment questionnaire for children 6 months through 6 years of age. (If from the lead questionnaire it is deemed necessary for the actual finger-stick Lead Test, then that will be required as well.) (Students entering pre-school and/or kindergarten received the health examination forms with their pre-school and/or kindergarten booklets.)
Also, effective July 15, 2005, the State of Illinois mandated DENTAL HEALTH EXAMS for students entering kindergarten, second, and sixth grades. For your convenience, we have included a Dental Examination Form in the brown envelopes of those families with a child entering second and sixth grades in the fall. (Students entering kindergarten received the form with their kindergarten booklet.)
Eye exams are now required for all children entering kindergarten or first grade for the first time, or for children who are transferring into Illinois for the first time. This law requires a comprehensive eye examination prior to starting school. An optometrist or ophthalmologist must do the examination. The exam may be done within one year prior to beginning school. This requirement begins with the 2008 school year and applies to all future years. (Additional copies of the necessary dental and health forms can be obtained in the school office.)
Whether you choose to have your child(ren) seen by your family physician and/or dentist, or utilize the Stephenson County Health Department/Crusader Clinic, please make appointments now so that your child(ren) will be able to present the required health records to school on the first day of school!
STEPHENSON COUNTY HEALTH DEPARTMENT
IMMUNIZATION CLINICS: For your information, the Stephenson County Health Department offers immunization clinics (by appointment only) every Thursday from 9:00 a.m. to 4:00 p.m. To make an appointment, call 815-235-8271. Anyone, regardless of income level, may receive immunizations at the Stephenson County Health Department. An administration fee of $15 each is charged for children immunizations. No charge will be assessed to those with Illinois Public Aid cards or All-Kids cards.
TB TESTING: TB testing is also provided by the Stephenson County Health Department free of charge to Stephenson County residents on Mondays and Tuesdays. Walk-in clients are accepted for TB skin testing. The times available are 9:00-11:00 a.m., and 1:00-3:00 p.m. Children must be under 18 years of age and must have parental consent
PEDIATRIC DENTAL: Furthermore, located at the Stephenson County Health Department is the Crusader Clinic. The Crusader Clinic has pediatric dental facilities and offers dental exams on a scheduled basis. Den-tal exams are available for children ages 2 through 18. To make an appointment for these services, please call 815-235-8271. Families with an Illinois Public Aid medical card or the All-Kids card will have no charge.
SCHOOL AND SPORTS PHYSICALS: School and sports physicals are available by appointment only. The cost is $30. All using this service are required to pay the fee. Accepted payment options are cash, check, VISA, or Master Card.
